What services do we provide?
We provide the following services: Medication Therapy Management, Annual Wellness Visits, Diabetes Education, Chronic Care Management, Transitional Care Management, Smoking Cessation Programs, Immunizations, prior authorization and refill requests and drug information support. These services are billable to Medicare B, Georgia Medicaid and most commercial insurance plans using incident-to codes.

Are there any other benefits to having a pharmacist around?
Of course! Isn’t it frustrating to see a patient after 3 months and you wonder why their glucose is uncontrolled? Well it turns out they only take their oral meds when they remember and they’ve been injecting their insulin in the wrong area. Having a pharmacist check in with them and make sure they get proper counseling on WHY they take medication every day and HOW to properly administer it will keep them healthier! Patients will appreciate the extra education so your Star rating (both CMS and Google) will go up!

We can provide in-person visits in the greater Atlanta area of Georgia, as well as remote visits for all of Georgia and Colorado.
